1. The Care Plan that I received from Britton House Community Mental Health Team which Dr Farid Latif and before that Dr Savito Mwanche of Lond Catlis Road Surgery as attached: MedCertDrWwanche29Nov2021, base on are on an outdated assessment of Dr Odesanya the Consultant Psychiatrist under whom I had assessments and recommended treatments for around two years as attached: CAREPLANDrOdesanya13September2021.pdf. I have not had a review of my mental health since then and so had to engage in the following discussions with the Store Manager Chiranjiv Rai of BP Lenham before your email came to me:


At 12.06 pm Mobile Phone rang, It was Chiran. The Back to Work Interview started. He asked whether I was well enough to return to work. I said I was 100 per cent fit, the doctor has said. He said we are only talking about your knee and feet injury. Are you taking any medications for it. I said I took Ibuprofen for the inflammation yesterday but today I have not taken it. He said do not record this conversation, that He was recording it. He asked whether I would like a phased return to work. I said No I would like to do my shifts as I used to 8 hours a day and I would be OK with 7.5 hours on the Till.He said because you are on probation you do not get paid for Company sickness absences during the probationary period. Your sick note was for one week from the 10 to the 17. I said that I went on selfcertified sickness from 2 till 9 February, then the doctore provided the Sick Note from 10-17 February. He then said that you must not send any emails to colleagues email addresses, if you have any issues you must phone the Store and speak to the Duty Manager and ask him to whom sick notes or other material should be sent. I said Vicky gave me her email address, I did not ask for it so I thought it was OK to send it. They were not responding to my telephone calls I spoke to Som and Ramesh and Chris etc. I will now delete their email addresses from my records. I do not want to send emails to anyone. He said when appropriate and comfortable you can arrange to send them to Line Manager. Regarding the Safety shoes I said I tried size 11 and then 12 but they were even heavier. So I bought Safety Shoes from Amazon and they were unsuitable and I resumed with the size 11 with insoles. He said you can purchase Safety shoes from anywhere such as Amazon and the Company will pay £30 towards it. He further said that any further sickness absences will result in the termination of employment and your probation will be extended by another 4 weeks. I replied that is fair. So your next shift he said is Saturday afternoon and I checked that the remaining shifts will be as on Refeixis 43 One. He confirmed. He ended by saying that a copy of this Action Plan will be given to you at my next shift. I said that will be Saturday at 2.00 pm, and the conversation interview ended at 12.30 pm.


2. Dr Mwanche and Dr Latif have not signed me off sick on mental health grounds specifically and no further sick note will be necessary. This is what I will argue with the Court of Central London in Claim E35YM660 on which Appeal is pending to the 21 June 2021 Order of the Court as attached: CentralLondonCountyCourtOrder21June2021(DistrictJudgeLigjhtman).pdf.


3. I do hope that we can resolve this matter that is under long-term dispute out of court, even at this eleventh hour that the clock is ticking towards.

I submitted my Sick Note to BP Lenham and Head Office for the period 10-17 February 2022 together with the Medications that are prescribed by me.


The GP Surgery has asked Dr Latif to provide a letter (S.PanigrahiDiagnosisTreatmentbyDrLatif16Feb2022.docs) to clarify my mental health condition as Paranoid Schizophrenia and Persistent Delusional Disorders among other abnormalities of the brain/mind for the purpose of me having to extend my Sick Leave beyond 17 February 2022 and until such time that I receive medical clearance for mental and physical health to resume work. My knee and heel injuries are not 100 percent healed either.


Please let me know whether a formal Sick Note is required to cover me for Statutory Sick Pay from 18 February 2022 onwards.
