I responded to Dr Latif’s Textphone message from the surgery with Dr Latif at 12.53 pm with: could you provide me with a Medical Certificate on the present diagnosis and treatment on my mental health with the current list of medications as was done by Dr Mwanche on 29 November 2021 please?
